when running PiP, if i mouse over the smaller picture do i have use of that char. ... say my tank is in the main window, and my healer is in the small window can i just put the mouse in the small window to heal the tank? or do i have to still switch windows to heal him.
Well, either way you have to switch windows. When using PiP mode, the active/foreground window is going to be the bigger window. If you want one of them to stay a picture window, you can make a smaller preset with always on top and manually use that, or use PiP lock (which is normally used for dragging the picture window to a new location)

i want to keep the healer in the smaller window and the tank in the bigger window and... to heal the tank all i have to do is mouse over the smaller window and BAM i control the healer now .. click on heal then mouse over the bigger window and BAM im tanking again?? or no

Not exactly. You can't just mouse over the other window to control it. You still have to actually switch windows, via alt-tab (in this case, it would have to be shift-alt-tab because the small window is in front of the active window) or WinEQ hotkey. It's still another window, just smaller and always on top.
If you always want that specific window to be small, Picture-in-Picture isn't necessarily what you want. PiP is designed so that whatever window you switch to becomes the large window, and the previous window becomes the small window. For what you want, it's better just to use the "tiny" preset (likely with mouse release turned off) or one like it.
