I editted the WinEQ default profile to change the window title. I clicked apply and ok to close out. I started EQ, but it's not changing the title to what I asked it to. I appended my charname to before the default "WinEQ..."

I went back in and the setting took and shows my charname in there, but it's not showing up in the titlebar. The WinEQ default profile is the only one I've editted and is the only one available at launchtime.

Did I miss a setting?

No, it's blocking changing the title for the first profile. I'll change it to not do that
